Description:
Primary aldosteronism should be suspected whenever a patient has the triad of hypertension, hypokalemia, and inappropriate renal potassium wasting. The biggest challenge is the differential diagnosis between the more common adrenal adenoma and bilateral hyperplasia. Because adrenal adenomas increase the circulating levels of 18-hydroxycorticosterone (18-OH B), serum levels of 18-OH B may be useful to differentiate them from bilateral adrenal hyperplasia. The cutoff point appears to be about 50 ng/dL of serum. Most values are around 100 ng/dL. The higher the level of this precursor, the greater the possibility that an adrenal adenoma is present.J Clin Endocrinol Metab. 97: 881-9, 2012.

The primary hyperoxalurias (PH) are a group of inherited disorders caused by defects in glyoxylate metabolism, with a combined incidence of about 1:60,000. To date, three primary hyperoxaluria have been described, designated as PH1, PH2, and PH3. All three forms follow an autosomal recessive mode of inheritance, and lead to increased oxalate levels in the body. The typical presentation is with recurring calcium oxalate renal stones. Calcium oxalate can also deposit in various tissues including the kidneys (nephrocalcinosis). End stage renal disease can occur from damage caused by the systemic oxalosis. The age of onset of symptoms ranges from infancy to adulthood; some adults may remain asymptomatic until the sixth decade.

Description:
5'-Nucleotidase (5'-NT) assay kit is for the determination of 5'-NT activity in human serum samples. 5'-NT is an enzyme catalyzing the hydrolysis of nucleoside-5'-mono- phosphates to nucleosides and inorganic phosphate. The enzyme is widely distributed in human and animal tissues. The activity present in sera is released from membranes of liver cells by bile salts and has been used as a marker for liver disease. Healthy subjects have a 5'-NT activity in the range of 0-10 U/L. Increased enzyme levels in sera are associated with certain forms of liver disease, such as intra- or extra-hepatic obstruction and particularly in cases of hepatic carcinoma and in mastectomy patients with recurrent metastases. The diagnostic value of 5'-NT has been shown to be superior to other liver enzymes, especially in cases of liver metastasis.

Description:
Kinase inhibitors such as IMATINIB mesylate (ST1571;GLEEVEC) are effective in the treatment of chronic Myelogenous Leukemia (CML) by selectively inhibiting BCR-ABL fusion protein. Most patients in chronic phase maintain durable responses; however, many in blast crisis fail to respond, or relapse quickly. ABL kinase domain mutations are the most identified mechanism associated with relapse. The molecular monitoring in the first few months of therapy may play a crucial role in detecting patients at high risk of resistance to therapy. The ABL kinase mutation assay sequences exons 4-9 of ABL1 kinase and will detect all drug-resistance mutations as recommended by guidelines including G250E, Y253H, E255K/V, V299L, F311L/V/I, T315I/A, F317L/V/I/C, A337T, F359V/I/C and P465S, which will aid in the selection of the appropriate kinase inhibitor therapies.

Description:
This allergen-specific IgE antibody test quantifies an individual’s IgE response to Acarus siro. It is an in vitro quantitative assay that is intended to be used in conjunction with other clinical information to aid in the diagnosis of allergic diseases [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. A definitive clinical diagnosis of allergy should not be based on the results of any single diagnostic method, but should be made by a trained healthcare provider after all clinical and laboratory findings have been evaluated. More specific information about this allergen can be found at http://www.phadia.com/en/products/allergy-testing-products/immunocap-allergen-information/mites/allergens/acarus-siro/ Reference 1. AccuType® CP, Clopidogrel CYP2C19 Genotype

|
|
Description:
This pharmacogenetic test may be used to help guide clopidogrel prescribing, especially in patients with acute coronary syndrome (ACS) who have undergone percutaneous coronary intervention (PCI) [1]. This test detects common variants in the CYP2C19 gene, the most validated genetic determinant of clopidogrel response.Clopidogrel is a prodrug that needs to be metabolized to its active form by cytochrome P450 2C19 (CYP2C19). Patients who carry a no-function allele of CYP2C19 have reduced clopidogrel activation, reduced platelet inhibition, and higher risk of major adverse cardiovascular events. Treatment outcomes may be improved in these patients with alternative antiplatelet therapies, such as prasugrel or ticagrelor. Therefore, CYP2C19 genotyping is helpful for assessing an individual's capability to metabolize clopidogrel and optimizing antiplatelet therapy [1].CYP2C19 genotype classifies individuals as ultrarapid, rapid, normal, intermediate, likely poor, poor, and indeterminate metabolizers. The Clinical Pharmacogenetics Implementation Consortium (CPIC) provides guidelines for using CYP2C19 status to help direct clopidogrel prescribing. Although the association between CYP2C19 genotype and clopidogrel response is most established in patients with ACS who have undergone PCI, growing evidence indicates a similar association in patients with neurovascular indications [1]. The FDA boxed warning indicates diminished antiplatelet effect in CYP2C19 poor metabolizers and availability of tests to identify these individuals [2].This is a targeted genotyping test and does not detect deletions or novel or rare variants of CYP2C19. CYP2C19 genotype does not identify all the variability in clopidogrel response. Other factors, such as age, body mass index, chronic kidney disease, diabetes mellitus, and concomitant use of certain proton pump inhibitors, may also influence clopidogrel response [1].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.Additional assistance in test selection and interpretation of results is available from our genomic science specialists by calling 1.866.GENE.INFO (1.866.436.3463).References1.Lee CR, et al. Warfarin (Coumadin®) therapy is associated with significant complications because of its narrow therapeutic index and large interpatient dosage variation necessary to achieve an optimal therapeutic response. This variation is due to both genetic and environmental factors. A promoter variant (-1639 G→A) of the Vitamin K epoxide complex subunit 1 (VCR) accounts for 25%-44% of this variability and variants of the cytochrome P enzyme C (SPCA) account for 10%-15% of this variability. Identification of these warfarin sensitive variants of the VKORC1 and the CYP2C9 genes may allow a more individualized therapy and reduced risk of bleeding complications.

Description:
Detects binding antibodies to the acetylcholine receptor (anti-AChR) by a quantitative radioimmunoassay in the sera of patients with Myasthenia Gravis (MG). About 80% to 85% of patients with generalized MG and 50% to 75% of patients with ocular MG have anti-AChR. Negative anti-AChR results are reflexed to testing for anti-MuSK (muscle specific receptor tyrosine kinase) and anti-LRP4 (low density lipoprotein receptor-related protein 4). Approximately 8% to 10% of all MG cases and 20% to 50% of the generalized MG cases who lack binding anti-AChR have anti-MuSK. Anti-LRP4 antibodies are detected in the serum of approximately 9.2% (range 2% to 50%) of MG patients who are negative for both binding anti-AChR and anti-MuSK.

Fewer than 1% of patients have blocking antibodies without binding antibodies. Blocking antibodies are present in about 50% of patients with MG, but rare in other conditions. Therefore, blocking antibodies have utility in ruling out a possible false positive binding assay and detecting the rare patient without AChR binding antibodies.

Description:
The G-nAChR autoantibody assay can be used to identify Autoimmune Autonomic Ganglionopathy (AAG) or paraneoplastic AAG in patients with autonomic neuropathy. Identification of paraneoplastic AAG in a patient is important, as there is an association between increased levels of G-nAChR antibodies and small-cell lung cancer (SCLC) or thymoma. Detection of paraneoplastic AAG, followed by resection and/or treatment of the patients’ tumor, can lead to a successful patient recovery. The G-nAChR autoantibody assay can be used as a serological diagnostic tool to differentiate patients suffering from immmunotreatment-responsive AAG from idiopathic autonomic neuropathy. Both conditions can present as acquired Autonomic Nervous System (ANS) dysfunction characterized by diffuse autonomic nervous system failure. The demonstration of AAG can lead to a focused immunological treatment and successful patient recovery. The G-nAChR antibody assay can also identify patients with limited forms of dysmotility, diabetic autonomic neuropathy, or postural tachycardia syndrome.

Patient should be cautioned to stop biotin consumption at least 72 hours prior to the collection of a sample. Estimated turnaround for results is 7 business days. If confirmation testing is required, the estimated time may be extended.

Description:
This test detects the presence of specific IgE against Acremonium kiliense. A positive test only indicates that a person is sensitized and potentially allergic to this allergen. A diagnosis of allergy requires clinical signs and symptoms that appear to be associated with exposure to the allergen in addition to the detection of markers of IgE sensitization [1].More specific information about this allergen can be found on the following website: https://www.thermofisher.com/phadia/us/en/resources/allergen-encyclopedia.htmlReference1. Description:
Actinomyces species are part of the normal flora of the mouth and female genital tract and produce infection primarily as endogenous opportunists. Actinomyces israelii is the primary etiologic agent of actinomycosis. The disease is manifested in three forms: Cervical, thoracic and abdominal lesions typically from sinus tracts which drain pus and "sulfur granules", which are microcolonies of Actinomyces and other bacteria. Pelvic Actinomyces israelii has been described in association with the use of intrauterine devices (IUDs).
