Description:
The Philadelphia Chromosome (Ph) is a translocation between chromosome 9 and 22 t(9; 22) (q34; Q11) that is found in more than 90-95% of chronic myeloid leukemia (CML), and in 20-25% of adult and 2-10% of childhood acute lymphoblastic leukemia (ALL). In CML, most translocations fall in the major breakpoint cluster region of the BCR gene, and result in either of two BCR-ABL1 mRNA molecules having an e13a2 junction (fusion of BCR exon 13 with ABL1 exon 2) or an e14a2 junction (fusion of BCR exon 14 with ABL1 exon 2) that encodes the p210 BCR-ABL1 fusion protein. In all, about two thirds of the BCR breakpoints fall in the minor breakpoint cluster region of the BCR gene, and the hybrid BCR-ABL1 transcript contains an e1a2 junction (fusion of BCR exon1 with AB11 exon2) which is translated as a p190 BCR-ABL1 fusion protein. Quantitative measurement of the changes of BCR-AB1 expression levels over time is important to monitor disease progression and response to targeted therapies. This test is a reverse-transcription pcr-based quantitative assay which detects these two major BCR-AB11 mRNA transcripts produced by the t(9; 22) chromosomal translocation (p210 and p190). BCR-ABL1 transcript levels are expressed as a percent ratio of BCR-ABL1 to the normalizing ABL1 transcript levels. For the p210 transcript associated with CML, quantitation is further adjusted to the international scale (IS) to allow comparison with other IS-compliant BCR-ABL1 assays. Optimal therapy in CML is associated with transcript levels below the major molecular response (MMR) milestone indicated by a BCR-ABL1/ABL1% (is) below 0.1.

Description:
The antiphospholipid syndrome (APS) is a systemic autoimmune disorder characterized venous and/or arterial thrombosis (intravascular blood clotting), thrombocytopenia, recurrent fetal loss, and moderate-to-high titers of antiphospholipid (aPL) antibodies, lupus anticoagulant (LA), anti-cardiolipin (aCL) antibodies and anti-Beta-2-glycoprotein I (aB2GPI) antibodies.aPL antibodies are autoantibodies that react with negatively charged phospholipids (e.g. Cardiolipin). They are frequently found in sera of patients with systemic lupus erythematosus (SLE) and related diseases and are typical for the development of secondary APS. The presence of antiphospholipid antibodies in patients with no other autoimmune diseases is characteristic of primary APS. Because aCL antibodies are found in 80-90% of patients with APS, this test is very sensitive for the diagnosis of the syndrome. However, the aCL test is not specific to APS because aCL antibodies can also be detected in other autoimmune diseases, following administration of several drugs, and in the aftermath of infectious diseases (e.g., syphilis, hepatitis C and infectious mononucleosis).B2GPI, also known as apolipoprotein H, is a 54 kDa serum cofactor (protein) that inhibits the intrinsic coagulation pathway and is involved in the regulation of blood coagulation. Detailed investigations about the nature of the cardiolipin-B2GPI complex have shown that B2GPI is the real autoantigen of anti-cardiolipin antibodies. In addition to being a prerequisite for binding to anti-cardiolipin antibodies, B2GPI has been identified as the primary antigen for these antibodies. Hence, an assay with a B2GPI bound solid phase will not bind antibodies from other disease states, e.g., in syphilis positive samples, and will show better overall disease specificity.A definite diagnosis of APS requires the presence of at least one clinical manifestation and one positive laboratory test result. The clinical criteria include thrombosis and one of the following three during pregnancy: one or more pregnancy losses after the 10th gestational week; one or more episodes of pre-term delivery before the 34th gestational week due to severe pre-eclampsia or severe placental insufficiency; or three or more recurrent pregnancy losses before the 10th gestational week. The laboratory criteria include the detection of IgG aCL or aB2GPI antibodies in moderate to high titers, in two different measurements at least 12 weeks apart; or detection of LA in two different measurements at least 12 weeks apart.Anti-B2GPI antibodies have been reported to bind to epitopes located primarily on domains 1, 4, and 5 of the B2GPI molecule. Importantly, the main B2GPI epitope specifically associated with antiphospholipid syndrome has been reported to be a cryptic and conformation dependent structure that involves different regions of domain 1. If testing is positive for aB2GPI IgG antibodies, reflex testing will be performed to determine if the antibodies are directed to B2GPI domain 1. A positive B2GPI domain 1, IgG antibody test result may be useful in identifying patients at increased risk for arterial and venous thrombosis and pregnancy morbidity.

Description:
Antiphospholipid syndrome (APS) is a systemic autoimmune disorder, characterized by recurrent arterial or venous thrombosis, and/or early and late miscarriages. Autoantibodies to phospholipids and to beta-2-glycoprotein 1 (B2GP1) play an important role in the pathogenesis of APS, and the measurement of these antibodies aids the diagnosis and management of the syndrome. Beta-2-glycoprotein 1, also called apolipoprotein H, is a 54 kDa glycoprotein with five domains that is present in plasma. The fifth domain contains a cluster of positively charged amino acids which is responsible for the binding to anionic phospholipids. APS patients can produce antibodies against several epitopes of the molecule. Antibodies to B2GPI are directed to one of the five domains of B2GPI, but only IgG antibodies directed to epitopes on one domain (called Domain 1 or D1) have been shown to be associated with increased risk of venous thrombosis. Even greater risk of venous thrombosis has been shown when phosphatidylserine/prothrombin antibodies are also present. This test should be used when B2GP1 IgG antibodies are present to determine whether the antibodies are directed to Domain 1. The presence of B2GPI Domain 1 antibodies should be confirmed by repeat testing at least 12 weeks apart.

Beta-2-Microglobulin, Random Urine with Creatinine

Description:
Beta-2 Microglobulin (B2M) is a low molecular weight protein that forms the light chain component of the histocompatibility antigen. It is synthesized by all nucleated cell types. It is an integral part of the class I MHC antigens and is present in all body fluids. B2M is filtered through the glomeruli of the kidney and is then reabsorbed and catabolized by the proximal tubular cells. In normal patients only trace amounts of B2M appear in the urine. Elevated urine B2M is seen in tubulo-interstitial disorders. Increased urine B2M is seen in cadmium exposure, diatrizoate, exercise, fever, nephrectomy, semen. There is evidence that monitoring B2M levels in HIV-infected individuals offers an independent predictor of progression to AIDS, leukemia and lymphoma.

Description:
Beta-2-microglobulin normally passes through the glomerulus into the proximal tubule where much of it is reabsorbed. Serum levels are therefore an index of glomerular function. When impaired, serum levels rise in inverse ratio to glomerular filtration rate. Increased amounts of beta-2-microglobulin are excreted in several renal disorders, e.g., Balkan nephropathy, heavy metal poisoning and renal tubular disease due to therapeutic agents. Serial levels of beta-2-microglobulin in serum and urine are used to evaluate transplant viability and anticipate rejection. Following a successful graft, serum levels decline toward normal. Increasing serum levels provide an early sign of rejection. Elevated levels are also noted in lymphproliferative disorders, neoplasms (malignant and benign), inflammatory disease, and autoimmune diseases such as systemic lupus erythematosus (SLE) and Sjögren's disease.
A significant deviation from the normal range may require further evaluation by your physician.

Description:
This test is an allergen-specific IgE antibody test that quantifies an individual’s IgE response to beta-lactoglobulin. It is an in vitro quantitative assay, which is intended to be used in conjunction with other clinical information to aid in the diagnosis of food allergy [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. Food-specific IgE tests are extremely sensitive. However, a positive test result only indicates that a patient is IgE sensitized to the food of concern. Many IgE-sensitized patients do not develop any symptoms when this food is ingested. A diagnosis of food allergy should only be made by a trained medical provider, after conducting a thorough clinical evaluation [2,3]. While food-specific IgE test results may contribute to that evaluation, they cannot replace it. In this regard, detection of food-specific IgE in serum provides evidence of IgE sensitization, but a history of clinical reactivity to the food of concern, is required to make a diagnosis of IgE-mediated food allergy.
