Description:
Increased serum CEA levels have been detected in persons with primary colorectal cancer and in patients with other malignancies involving the gastrointestinal tract, breast, lung, ovarian, prostatic, liver and pancreatic cancers. Elevated serum CEA levels have also been detected in patients with nonmalignant disease, especially patients who are older or who are smokers. CEA levels are not useful in screening the general population for undetected cancers. However, CEA levels provide important information about patient prognosis, recurrence of tumors after surgical removal, and effectiveness of therapy.

Description:
This pharmacogenetic test may be used to help guide clopidogrel prescribing, especially in patients with acute coronary syndrome (ACS) who have undergone percutaneous coronary intervention (PCI) [1]. This test detects common variants in the CYP2C19 gene, the most validated genetic determinant of clopidogrel response.Clopidogrel is a prodrug that needs to be metabolized to its active form by cytochrome P450 2C19 (CYP2C19). Patients who carry a no-function allele of CYP2C19 have reduced clopidogrel activation, reduced platelet inhibition, and higher risk of major adverse cardiovascular events. Treatment outcomes may be improved in these patients with alternative antiplatelet therapies, such as prasugrel or ticagrelor. Therefore, CYP2C19 genotyping is helpful for assessing an individual's capability to metabolize clopidogrel and optimizing antiplatelet therapy [1].CYP2C19 genotype classifies individuals as ultrarapid, rapid, normal, intermediate, likely poor, poor, and indeterminate metabolizers. The Clinical Pharmacogenetics Implementation Consortium (CPIC) provides guidelines for using CYP2C19 status to help direct clopidogrel prescribing. Although the association between CYP2C19 genotype and clopidogrel response is most established in patients with ACS who have undergone PCI, growing evidence indicates a similar association in patients with neurovascular indications [1]. The FDA boxed warning indicates diminished antiplatelet effect in CYP2C19 poor metabolizers and availability of tests to identify these individuals [2].This is a targeted genotyping test and does not detect deletions or novel or rare variants of CYP2C19. CYP2C19 genotype does not identify all the variability in clopidogrel response. Other factors, such as age, body mass index, chronic kidney disease, diabetes mellitus, and concomitant use of certain proton pump inhibitors, may also influence clopidogrel response [1].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.Additional assistance in test selection and interpretation of results is available from our genomic science specialists by calling 1.866.GENE.INFO (1.866.436.3463).References1. Description:
Measurement of serum 25-OH vitamin D concentrations provide a good index of circulating vitamin D activity in patients not suffering from renal disease. Lower than normal 25-OH vitamin D levels can result from a dietary deficiency, poor absorption of the vitamin or impaired metabolism of the sterol in the liver. A 25-OH vitamin D deficiency can lead to bone diseases such as rickets and osteomalacia. Above normal levels can lead hypercalcemia. This assay employs liquid chromatography tandem mass spectrometry to independently measure and report the two common forms of 25-hydroxy vitamin D: 25OH D3 - the endogenous form of the vitamin and 25OH D2 - the analog form used to treat 25OH Vitamin D3 deficiency.

Cardiometabolic Disease Assessment (CMDA) Panel

Description:
This panel combines tests used in the diagnosis and management of cardiometabolic disease elements, including metabolic syndrome, prediabetes, diabetes, thyroid conditions, kidney, fatty liver, and cardiovascular disease risk. This panel can help identify the presence and progression of cardiometabolic disease by assessing metabolic state and downstream effects on the liver, kidneys, and heart. It is estimated that as many as 45% of Americans have a chronic disease; more than half of older adults have multiple chronic conditions [1]. The related chronic cardiometabolic conditions are characterized by initial stages that may remain clinically silent for years.Medical societies, like the American Heart Association, recognize the interrelated impacts of metabolic risk factors (ie, obesity and diabetes), chronic kidney disease, and cardiovascular disease, and the gap in early screening, prevention, and management of these chronic conditions [2,3]. Rising rates of metabolic dysfunction, obesity, and type 2 diabetes mellitus (T2DM) also contribute to increased prevalence of nonalcoholic fatty liver disease (NAFLD), also known as metabolic dysfunction-associated steatotic liver disease (MASLD) [4]. Thyroid health may either be impacted by or exacerbate cardiometabolic conditions, such as CHD, NAFLD, and CKD [5-8].The Cardiometabolic Disease Assessment Panel includes guideline-recommended testing that spans interrelated areas of cardiometabolic disease to determine the extent of metabolic dysfunction (glycemic control and advanced lipid assessment), identify whether thyroid health contributes to or is altered by metabolic dysfunction, and adverse impacts to heart, liver, and kidney function [9-13]. Identification of cardiometabolic disease presence can help implement personalized treatment strategies that can prevent or delay disease progression and related complications. The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1. Description:
A plasma acylcarnitine profile is one of the most powerful and accurate methods to diagnose inborn errors of fatty acid metabolism and organic acidemias. In newborn screening, newborns have sufficient free carnitine provided through the placenta from the mother to make acylcarnitine profiling alone sufficient to rule out these inborn errors. Unfortunately, after the newborn period, at the time of presentation and diagnosis, many patients with these disorders have become chronically depleted of free carnitine due to the inability to recycle esterified carnitine. In this situation, the acylcarnitine profile may be normal with all values in the lower end of the normal range despite the patient being affected with a disorder of fatty acid oxidation or an organic acidemia. Recently, we had just such a case with a patient known to have methylmalonic acidemia because of organic acid analysis who initially had a normal acylcarnitine profile. In such cases, a carnitine analysis would have shown extremely low free carnitine and very elevated carnitine esters and would have prompted carnitine supplementation of the patient followed by further periodic evaluations. As clinicians come to rely increasingly on acylcarnitine profiling to diagnose inborn errors of metabolism, it will be important to assay both carnitine (free and esterified) simultaneously with acylcarnitines for the initial evaluation of children for inborn errors of metabolism. This approach would prevent false negative results and false reassurance that could occur if acylcarnitines alone were ordered.

Description:
This test is an allergen-specific IgE antibody test that quantifies an individual’s IgE response to casein. It is an in vitro quantitative assay, which is intended to be used in conjunction with other clinical information to aid in the diagnosis of food allergy [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. Food-specific IgE tests are extremely sensitive. However, a positive test result only indicates that a patient is IgE sensitized to the food of concern. Many IgE-sensitized patients do not develop any symptoms when this food is ingested. A diagnosis of food allergy should only be made by a trained medical provider, after conducting a thorough clinical evaluation [2,3]. While food-specific IgE test results may contribute to that evaluation, they cannot replace it. In this regard, detection of food-specific IgE in serum provides evidence of IgE sensitization, but a history of clinical reactivity to the food of concern, is required to make a diagnosis of IgE-mediated food allergy. Moreover, several forms of food hypersensitivity are not associated with the presence of food-specific IgE in serum. More specific information about this allergen can be found at http://www.phadia.com/en/Products/Allergy-testing-products/ImmunoCAP-Allergen-Information/Food-of-Animal-Origin/Allergen-Components/nBos-d-8-Casein-Milk/ References 1. Description:
The clinical utility of food-specific IgG4 tests has not been clearly established. Physicians sometimes use allergen- specific IgG4 to evaluate patients prior to or during immunotherapy. These tests can be used in special clinical situations. This test should only be ordered by physicians who recognize the limitations of the test.
