Description:
Whole food allergen IgE tests are very sensitive but some patients with positive results do not have a true food allergy. Food component IgE tests offer more specific information that better predicts the likelihood that a true food allergy exists and other details of potential clinical relevance. A final food allergy diagnosis should always be based on a combination of IgE test results, a physical examination, and clinical history.

Description:
This test detects the presence of specific IgE against cashew nuts. A positive test only indicates that a person is sensitized and potentially allergic to this allergen. A diagnosis of allergy requires clinical signs and symptoms that appear to be associated with exposure to the allergen in addition to the detection of markers of IgE sensitization [1].More specific information about this allergen can be found on the following website: https://www.thermofisher.com/phadia/us/en/resources/allergen-encyclopedia.htmlReference1. Description:
This test measures specific IgE antibodies to cashew nuts with reflex to cashew nut component Ana o 3 if the IgE result is positive. Allergen-specific serum IgE testing is considered comparable to skin testing and may be preferred in some clinical situations. However, a positive test result only indicates that a patient is sensitized to the allergen of concern. Many IgE-sensitized individuals do not develop any symptoms when exposed to the allergen. A diagnosis of allergy should only be made by a trained medical provider after conducting a thorough clinical evaluation [1].More specific information about allergens included in this panel may be found on the Quest Diagnostics Test Directory.Reference1. Cat Dander Component Panel

Description:
Like respiratory allergies, animal allergies pose a significant risk for the development of asthma and rhinitis. Blood IgE tests using whole allergen extracts from cats provide a sensitive screening method to diagnose pet sensitization. In addition, the cat dander component panel is an IgE blood test using individual recombinant proteins from cats, improving specificity compared to animal extract assays. Thus, the cat dander component panel helps differentiate an actual cat allergy from a cross-reactive pet allergy.

Description:
This allergen-specific IgE antibody test quantifies an individualโs IgE response to cat dander. It is an in vitro quantitative assay that is intended to be used in conjunction with other clinical information to aid in the diagnosis of allergic diseases [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. A definitive clinical diagnosis of allergy should not be based on the results of any single diagnostic method, but should be made by a trained healthcare provider after all clinical and laboratory findings have been evaluated. More specific information about this allergen can be found at http://www.phadia.com/en/products/allergy-testing-products/immunocap-allergen-information/epidermals-and-animal-proteins/allergens/cat-dander/ Reference 1. Description:
This panel may aid in evaluating catecholamine-producing pheochromocytomas and paragangliomas (PPGLs). However, measurements of plasma free or urinary fractionated metanephrines (ie, the O-methylated catecholamine metabolites) are preferred for the initial biochemical evaluation of PPGLs because of their overall high diagnostic sensitivity [1]. To adjust for the concentration differences in random urine specimens, results are expressed as catecholamine to creatinine ratios. Most PPGLs secrete catecholamines and can cause catecholamine excess, resulting in hypertension, arrhythmia, and hyperglycemia. Left untreated, PPGLs often lead to life-threatening cardiovascular complications. The estimated prevalence of PPGLs is 0.05% to 0.1% in adults with hypertension and 1.7% in children with hypertension [2]. Recognizing the possibility of a PPGL and performing appropriate biochemical testing are crucial for the diagnosis [1]. Plasma or urine catecholamine levels may be used, in addition to metanephrine levels, to provide additional information during the initial biochemical evaluation of PPGLs (eg, when detecting dopamine-secreting paragangliomas) [2].Intermittent or insignificant secretion of catecholamines by some PPGLs may not increase catecholamine measurements. Physiological stress and certain medications (eg, acetaminophen, labetalol, and sotalol) may increase catecholamine measurements [1].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.โ โ โ โ โ โ โ References1. Description:
Testing for CD117/c-kit expression may be a useful tool for the identification of several malignant neoplasma expressing c-kit, including gastrointestinal stromal tumors (GISTS), mast cell diseases, acute myeloid leukemia (AML), small cell lung carcinoma (SCLC), and Ewing's sarcoma. It may also aid in differentiating gastrointestinal stromal tumors (GISTS) from other intra-abdominal mesenchymal tumors. Recent studies have indicated that the tyrosine kinase inhibitor, STI571 (Gleeva), may be effective in treating CD117/c-kit positive tumors. Therefore, testing tumors for CD117/c-kit immunoreactivity can assist in both diagnosis and the prediction of drug sensitivity to tyrosine kinase inhibitor STI571 treatment.

CD16A (FCGR3A) F158V Polymorphism Analysis - The CD16A (FCGR3A) gene encodes for an Fc-gamma receptor on human white blood cells which plays an important role in antibody dependent cell-mediated cytotoxicity (ADCC). In cancer patients, differential clinical response to therapeutic monoclonal antibodies has been reported to correlate with the genotype of the CD16A (FCGR3A) gene (PMID: 23286345, PMID: 30374078, PMID 33480963). The different genotypes (V/V, F/V, or F/F) result from a single nucleotide polymorphism, dbSNP rs396991, where a T>G substitution changes phenylalanine (F) to valine (V) at position 158 (P08637 VAR 003960).

Description:
This immunoassay is a first-line screening test to evaluate neoplasms of unknown origin and to help determine whether a cell is derived from a hematopoietic origin. In benign tissue or cell blocks, CD45 can be used as a marker of inflammatory cells. CD45 may be used in the evaluation of lymphomas [1]. The CD45 immunohistochemical stain uses an antibody against a transmembrane glycoprotein expressed on most nucleated hematopoietic cell types. It is used to evaluate cells of hematopoietic lineage. Positive staining is generally indicated by membranous cellular immunoreactivity, but sometimes cytoplasmic immunoreactivity is also seen [1]. The CD45 test will identify most nucleated cells of hematopoietic lineage, including granulocytes, basophils, lymphocytes, mast cells, macrophages, monocytes, and plasma cells. It will not stain platelets, erythrocytes, megakaryocytes, or erythrocyte precursors. Other cell types, including dendritic cells and thymocytes, may also show immunoreactivity to CD45 [1]. In the evaluation of analysis of hematopoietic tumors, the CD45 immunostain will stain neoplastic hematopoietic cells as well as infiltrating non-neoplastic inflammatory cells [1]. This test should be interpreted in the context of pertinent clinical and family history and physical examination findings. Reference 1. Rekhtman N, et al. Quick Reference Handbook for Surgical Pathologists. 2nd ed. New York, NY: Springer;2011:34.
