Description:
This test is used in the diagnosis of Smith-Magenis syndrome (SMS), a genetic disorder caused by a deletion at chromosome 17p11.2. SMS is characterized by distinctive facial features, skeletal malformations, developmental delay, cognitive impairment, behavioral abnormalities, sleep disturbances, and childhood obesity [1].Infants born with SMS often have feeding difficulties, hypotonia, and failure to thrive. As the child ages, characteristic facial features and behavioral abnormalities (including sleep disturbances and self-injurious behaviors) become more apparent. Developmental delay, intellectual disability, speech impairment, hearing loss, and physical abnormalities involving the eye, ear, and/or short stature may also be present [1].Although the 17p11.2 region contains multiple genes, RAI1 was found to be responsible for most clinical manifestations of SMS. Other than deletions at chromosome 17p11.2, pathogenic variants in RAI1 also lead to SMS [1]. Therefore, a negative FISH result does not rule out SMS. Tests such as chromosomal microarray and gene sequencing are often used to identify gene changes and differentiate SMS from other genetic disorders with similar clinical features. The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.Reference1. Description:
Rearrangements of the T-cell receptor alpha/delta TRA/D are recurrent in both mature and immature T-cell neoplasms. Many of these TRA/D rearrangements are cryptic at the conventional cytogenetic level and therefore are detected mainly by molecular testing and FISH. The majority of these rearrangements are translocations that juxtapose a transcription factor gene to the TRA/D locus leading to its aberrant expression in developing thymocytes.

Description:
The most common prognostically established cytogenetic abnormalities in CLL/SLL are 13q14 deletion, +12, TP53 (17p) deletion and ATM (11q) deletion. Although all these aberrations can be detected using the CLL/SLL FISH panel, trisomy 12 and 13q deletion status can be ordered separately, 13q deletion is associated with a favorable prognosis in CLL/SLL and it is also seen in other lymphoproliferative disorders. Trisomy 12 is strongly associated with CLL/SLL and less frequently with other lymphoproliferative disorders. In CLL/SLL patients, trisomy 12 is often characterized by a lymphocytic proliferation of cells with atypical immunophenotype and lymphocyte morphology and an intermediate prognosis.

Description:
This fluorescence in situ hybridization (FISH) assay detects a partial deletion of the Wolf-Hirschhorn syndrome (WHS) critical region (WHSCR) in chromosome 4p16.3. The results of this test may aid in the diagnosis of WHS.WHS is a rare chromosomal deletion disorder characterized by distinct craniofacial features ("Greek warrior helmet" appearance), prenatal and postnatal growth deficiency, microcephaly, seizure, hypotonia, and intellectual disability [1]. Patients with WHS may also have congenital malformations in the heart, kidney, genitourinary tract, and skeleton. The pathogenesis of WHS involves multiple genes, and the clinical severity generally correlates with deletion size. The deletions associated with WHS can be de novo (85%-90%) or inherited from parental balanced rearrangements (10%-15%) [1].A heterozygous deletion of the WHSCR on chromosome 4p16.3 detected by a FISH test supports the diagnosis of WHS [2]. However, because the FISH test is a targeted deletion analysis, a negative result of the FISH test does not rule out other deletions associated with the WHS or other deletion syndromes with similar clinical features. Chromosomal microarray detects almost all deletions of the WHSCR and can define the size of the deletion, and thus is considered the preferred test for WHS [2].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1. FLT3 Mutation Analysis

|
Description:
FLT3 (fms-like tyrosine kinase-3) is a receptor tyrosine kinase that plays a significant role in cell survival, proliferation, and differentiation of hematopoietic stem cells. Mutations of FLT3 represent one of the most frequent molecular mutations in acute myeloid leukemia (AML) and are reported in 25-30% of the patients. These mutations predominantly include FLT3 internal tandem duplications (ITD) which occur in the region of the gene encoding the Juxtamembrane region of the protein and point mutations in the tyrosine kinase domain (TKD). Patients with a high mutant allelic ratio (>0.5) are more likely to be responsive to FLT3 inhibition compared to those with low allelic ratio but also with poor prognosis due to high relapse risk. The TKD mutations most frequently involve mutation or deletion of codons D835 or I836. Both ITD and TKD mutations constitutively activate FLT3 kinase activity without the need for the ligand and hence promote malignancy. AML patients with FLT3 internal tandem duplication (ITD) mutations have poor prognosis, high relapse rates, and reduced overall survival. Patients with FLT3 mutations could be eligible for FLT3-targeted therapy regimens.

Description:
Folic acid deficiency is common in pregnant women, alcoholics, in patients whose diets do not include raw fruits and vegetables, and in people with structural damage to the small intestine. The most reliable and direct method of diagnosing folate deficiency is the determination of folate levels in both erythrocytes and serum. Low folic acid levels, however, can also be the result of a primary vitamin B12 deficiency that decreases the ability of cells to take up folic acid.
A significant deviation from the normal range may require further evaluation by your physician.
Alternative Names(s): Folic Acid
Includes:
Folate (Folic Acid)
Patient Instructions:
Folate assays of samples from patients receiving therapy with certain pharmaceuticals (eg, methotrexate or leucovorin), are contraindicated because of the cross-reactivity of folate binding protein with these compounds. Some tests in this panel may exhibit interference when sample is collected from a person who is consuming a supplement with a high dose of biotin (also termed as vitamin B7 or B8, vitamin H or coenzyme R). Patient should be cautioned to stop biotin consumption at least 72 hours prior to the collection of a sample.

Description:
FSH/LH testing is used in the workup of infertility (for both men and women). Used also to investigate low sperm count, menstrual irregularities, pitutary disorders, diseases involving the testes or ovaries. May be used to determine the cause of early or delayed puberty. FSH and LH are secreted by the anterior pituitary in response to gonadotropin-releasing hormone (GNRH) secreted by the hypothalamus. In both males and females, FSH and LH secretion is regulated by a balance of positive and negative feedback mechanisms involving the hypothalamic-pituitary axis, the reproductive organs, and the pituitary and sex steroid hormones. FSH and LH play a critical role in maintaining the normal function of the male and female reproductive systems. Abnormal FSH levels with corresponding increased or decreased levels of LH, estrogens, progesterone, and testosterone are associated with a number of pathological conditions. Increased FSH levels are associated with menopause and primary ovarian hypofunction in females and primary hypogonadism in males. Decreased levels of FSH are associated with primary ovarian hyper-function in females and primary hypergonadism in males. Normal or decreased levels of FSH are associated with polycystic ovary disease in females. In males, LH is also called interstitial cell-stimulating hormone (ICSH). Abnormal LH levels with corresponding increased or decreased levels of FSH, estrogens, progesterone, and testosterone are associated with a number of pathological conditions. Increased LH levels are associated with menopause, primary ovarian hypofunction, and polycystic ovary disease in females and primary hypo-gonadism in males. This in vitro allergen-specific IgE panel is used to quantitatively measure an individual's IgE response to 15 different foods that are commonly associated with allergies. These foods include almond, cashew nut, codfish, cow's milk, egg white, hazelnut, peanut, salmon, scallop, sesame seed, shrimp, soybean, tuna, walnut, and wheat. If the IgE result for egg white, cow's milk, or peanut is abnormal, the corresponding allergen component panel will be performed. This IgE panel is intended to be used in conjunction with other clinical information to aid in the diagnosis of food allergies [1].While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include (1) the presence of widespread skin disease, (2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, (3) uncooperative patients, and (4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1].Food-specific IgE tests are extremely sensitive. However, a positive test result only indicates that a patient is IgE-sensitized to the food of concern. Many IgE-sensitized individuals do not develop any symptoms when the food is ingested. A diagnosis of food allergy should only be made by a trained medical provider after conducting a thorough clinical evaluation [2,3]. While food-specific IgE test results may contribute to that evaluation, they cannot replace it. Moreover, several forms of food hypersensitivity are not associated with the presence of food-specific IgE in serum. The results of this panel should be interpreted in the context of pertinent clinical and family history and physical examination findings. More specific information about each allergen included in this panel may be found on the Quest Diagnostics Test Directory.References1. Bernstein IL, et al. IgE-assays are the best-studied and most accepted serologic tests for evaluating food allergy status. However, some clinicians also use IgG tests when evaluating individuals for food allergies, sensitivities, or tolerance. Food-specific IgG is generally believed to be a normal physiological response to food exposures. Therefore, many professional organizations, including the American Academy of Allergy, Asthma & Immunology, the Canadian Society of Allergy and Clinical Immunology, and the European Academy of Allergy and Clinical Immunology, recommend against using IgG testing to diagnose food allergies or food intolerances/sensitivities [1-3].
We offer IgG food tests to support providers who use them in their evaluation of patients experiencing adverse reactions to foods.
