Description:
Detection of antibodies to gliadin, one of the major protein components of gluten, is a sensitive assay useful in diagnosing celiac disease. However, gliadin antibodies may be found in individuals without celiac disease; thus gliadin antibody assays are less specific than assays measuring antibodies to endomysium and transglutaminase. Recent work has revealed that gliadin-reactive antibodies from celiac patients bind to a very limited number of specific epitopes on the gliadin molecule. Further, deamidation of gliadin results in enhanced binding of gliadin antibodies. Based on this information, assays using deamidated gliadin peptides bearing the celiac-specific epitopes have much higher diagnostic accuracy for celiac disease when compared to standard gliadin antibody assays.

Glial Fibrillary Acidic Protein (GFAP)

Description:
Glial fibrillary acidic protein (GFAP) is a protein expressed in astrocytes, cells that support neurons and help form the blood-brain barrier, found within the central nervous system (CNS) and is involved in several important CNS processes. It is a marker of astrogliosis and is upregulated in the brain as a result of attempts at repair and the formation of a glial scar. Glial scarring occurs as a result of several neurodegenerative processes, including Alzheimer's disease, multiple sclerosis, and neuromyelitis optica. It can also be seen after damaging pathological events such as traumatic brain injury and stroke [1]. Increased presence of GFAP can also be observed in association with brain tumors such as glioblastoma multiforme [2]. GFAP expression is known to decrease in disorders such as schizophrenia, bipolar disorder, and depression [3].1. Nature Reviews Neurology 18(3):158-172.2. J Neuro Sci 35 (1):147-155.3. Description:
Gaucher disease (GD) is one of the most frequent lysosomal disorders with a prevalence of 1:50,000 in the general population. It is characterized by a deficiency in the activity of the enzyme acid B-glucosidase, which leads to accumulation of glucocerebroside within lysosomes of macrophages, leading to hepatosplenomegaly, bone marrow suppression, and bone lesions. Depending on onset and symptoms, GD can be classified as non-neuronopathic, which is the mildest and most common phenotype in GD; the acute, neuronopathic form, which represents the severest form of the disease and is fatal within a few years; and the sub- acute, chronic neuronopathic form. Non-neuronopathic GD is the most common disease manifestation in the Ashkenazi Jewish population and with a frequency of up to 1:1,000. Genotype-phenotype correlations have been described in the literature. The most common examples are the mutation N370S usually detected in patients with non-neuronopathic GD carrying a milder burden of disease whereas e.g., the mutation L444P is more frequently seen in the more severe neuronopathic form of GD.Recently, Lyso GL-1 (glucosyl-sphingolipid) has emerged as a specific biomarker for GD. It is generated from the accumulated substrate glucosylceramide (GL-1) by the action of acid ceramidase. Being soluble in nature, Lyso GL-1 exits the lysosomal system and accumulates in other organs. The molecule is postulated to play a role in various pathophysiological mechanisms including neuronal injury in Gaucher patients by deregulating signal transduction pathways via the inhibition of protein kinase C.

Gluten (f79) IgE

Description:
This test is an allergen-specific IgE antibody test that quantifies an individual’s IgE response to gluten. It is an in vitro quantitative assay, which is intended to be used in conjunction with other clinical information to aid in the diagnosis of food allergy [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. Food-specific IgE tests are extremely sensitive. However, a positive test result only indicates that a patient is IgE sensitized to the food of concern. Many IgE-sensitized patients do not develop any symptoms when this food is ingested. A diagnosis of food allergy should only be made by a trained medical provider, after conducting a thorough clinical evaluation [2,3]. While food-specific IgE test results may contribute to that evaluation, they cannot replace it. In this regard, detection of food-specific IgE in serum provides evidence of IgE sensitization, but a history of clinical reactivity to the food of concern, is required to make a diagnosis of IgE-mediated food allergy. Moreover, several forms of food hypersensitivity are not associated with the presence of food-specific IgE in serum. More specific information about this allergen can be found at http://www.phadia.com/en/products/allergy-testing-products/immunocap-allergen-information/food-of-plant-origin/grains/gluten-/ References 1. Description:
The mucopolysaccharidoses (MPSs) are a family of inheritable disorders caused by deficiency of lysomal enzymes required to degrade mucopolysaccharides, also known as glycosaminoglycans (GAGs). The undegraded or partially degraded gags are stored in lysosomes and excreted in the urine. The quantity of excreted urinary gags is age-dependent. Infants secrete more GAGs than adults. Normal urine contains primarily chondroitin sulfate with small quantities of heparin sulfate and dermatan sulfate. Once mucopolysaccharidoses is diagnosed by total GAG analysis, a differential diagnosis based upon the abnormal distribution of sulfated GAGs in urine must be performed. Differential diagnosis is a requirement because many of the various enzyme deficiencies share similar clincal features. These features include a chronic and progressive course, multi-system involvement and organomegaly. Hearing, vision, cardiovascular function and joint mobility are affected. Profound mental retardation is found in the Hurler, Hunter and San Filippo syndromes (MPS types I, II and III), but normal intellectual functioning is retained in other MPSs and some mildly affected Hunter patients.

Description:
This test detects the presence of specific IgE against Glycyphagus domesticus. A positive test only indicates that a person is sensitized and potentially allergic to this allergen. A diagnosis of allergy requires clinical signs and symptoms that appear to be associated with exposure to the allergen in addition to the detection of markers of IgE sensitization [1].More specific information about this allergen can be found on the following website: https://www.thermofisher.com/phadia/us/en/resources/allergen-encyclopedia.htmlReference1.
