Description:
Key clinical use and differentiatorsMeasurement of Insulin Autoantibody (IAA) is used to: 1. Screen for presymptomatic type 1 diabetes (T1D) in individuals with a family history of T1D or those known to have a high-risk HLA genotype, 2. Distinguish between T1D, type 2 diabetes mellitus (T2D), and atypical forms of diabetes such as latent autoimmune diabetes in adults (LADA). [1] Insulin Autoantibody (IAA) is used to help predict the need for insulin treatment in adults with diabetes who have not received treatment with insulin. [1]Intended Clinical UseThis test is crucial for differentiating between T1D and T2D. It is important to note that classifying diabetes type can be challenging at the time of presentation, and misdiagnosis can occur in approximately 40% of adults with new T1D. [1]Identifying individuals affected by stage 1 T1D is important for applying eligibility criteria to drugs, such as Teplizumab, which have demonstrated the ability to delay the transition of stage 1 to stage 3 T1D in clinical trials [2,3]IAA has some distinctive features, including that usually it is the first detected autoantibody in young children, [4] its appearance is more common in younger children [5] and its frequency of appearance declines with age. IAA is not informative for individuals treated with insulin, who often develop antibodies in response to injected insulin. [6]LimitationsT1D is classified into three stages, with stages 1 and 2 considered presymptomatic. Since the presence of stage 1 and 2 T1D requires at least two islet autoantibodies, relying solely on IAA as a screening test limits the ability to make an accurate diagnosis. Professional organizations, including the American Diabetes Association (ADA), recommend using tests for all four islet cell autoantibodies: IAA, glutamic acid decarboxylase (GAD65), Zinc Transporter 8 (ZnT8), and islet cell antigen 2 antibody (IA-2).[1]References1. Description:
The lack of insulin causes the level of sugar in your blood to become abnormally high. When your body does not have enough insulin, it is hard for sugar to enter your cells. Sugar builds up in the blood. Too much sugar in your blood can cause many problems. These problems can be life-threatening if they are not treated. However, proper treatment can control your blood sugar level.
A significant deviation from the normal range may require further evaluation by your physician.
Includes:
Insulin.
Patient Instructions:
Fast for 12 hours prior to specimen collection. Fasting is defined as no consumption of food or beverage other than water for at least 12 hours before testing.

Description:
Insulin is a key hormone involved in the control of blood glucose and regulation of fatty acid metabolism. Insulin measurement is primarily used to evaluate the cause of hypoglycemia. Inappropriately elevated insulin in blood (hyperinsulinemia) is associated with hypoglycemia(1) or in disorders linked to hyperglycemia, such as metabolic syndrome(2). Causes of hyperinsulinemia include insulinoma, insulin resistance, noninsulinoma pancreatogenous hypoglycemia syndrome, insulin antibodies, surreptitious insulin administration or other drug-induced hyperinsulinism (e.g., sulfonyl urea), incretin effects after bariatric surgery, and congenital hyperinsulinism. Insulin (test code 561) immunoassay can be used to evaluate the etiology of hypo- or hyperglycemia but is not equivalent to the LC/MS/MS test. Combined with LC/MS/MS measurement of C-peptide in the Cardio IQ Insulin Resistance Panel with Score (test code 36509), the assay can also help evaluate the likelihood that an individual has clinically significant insulin resistance or help in differential diagnosis (e.g., insulinoma versus exogenous insulin administration(3). The results of this test should be interpreted in the context of pertinent clinical history and physical examination findings.References1. Cryer PE, Axelrod L, Grossman AB, et al. Insulin-Like Growth Factor I (IGF-I, LC/MS)

|
|
|
Description:
IGF-1, also known as somatomedin-C, is a polypeptide hormone produced by the liver and other tissues in response to growth hormone release by the pituitary gland. IGF-1 effects growth promoting activity and glucose metabolism (insulin-like activity) in most human tissues. It is carried in blood bound to a carrier protein which prolongs its half-life; therefore making serum levels more constant than growth hormone while correlating with "average" circulating levels of growth hormone. In short, IGF-1 is a surrogate marker of growth hormone levels and activity.
Measuring IGF-I is useful in several growth-related disorders. Dwarfism caused by deficiency of growth hormone (hypopituitarism) results in decreased serum levels of IGF-I, while acromegaly (growth hormone excess) results in elevated levels of IGF-I. IGF-I measurements are also helpful in assessing nutritional status; levels are reduced in undernutrition and restored with a proper diet.
A significant deviation from the normal range may require further evaluation by your physician.

Intrinsic Factor Blocking Antibody

Description:
Intrinsic factor is a protein that is produced by the parietal cells of the stomach. It binds to vitamin B12, thereby allowing absorption of the vitamin by intestinal cells. In pernicious anemia, an autoimmune disorder, autoantibodies are formed against intrinsic factor leading to its deficiency. The lack of intrinsic factor then causes malabsorption of vitamin B12 and subsequent anemia. Autoantibodies can also be formed directly against the parietal cells which similarly leads to inadequate intrinsic factor activity and vitamin B12 deficiency.
Patient Instructions:
Samples should not be collected from a patient who has received Vitamin B12 injection therapy within the last week.

Description:
The Iron Test with Total iron-binding capacity (TIBC) can detect how much iron is in the blood. Serum iron quantification is useful in confirming the diagnosis of iron-deficiency anemia or hemochromatosis. The measurement of total iron binding in the same specimen may facilitate the clinician's ability to distinguish between low serum iron levels caused by iron deficiency from those related to inflammatory neoplastic disorders. The assay for iron measures the amount of iron which is bound to transferrin. The total iron binding capacity (TIBC) measures the amount of iron that would appear in blood if all the transferrin were saturated with iron. It is an indirect measurement of transferrin concentrations but expressed as an iron measurement. To obtain the percent saturation, the serum iron is divided by the TIBC which gives the actual amount of saturated transferrin. The percent saturation is low in iron deficiency and high in iron storage diseases.
A significant deviation from the normal range may require further evaluation by your physician.

Iron, TIBC and Ferritin Panel

Description:
This serum iron study panel may help diagnose iron deficiency or overload. Because ferritin level can be affected by clinical conditions other than iron disorders, the measurement of transferrin saturation-calculated from serum iron level and total iron binding capacity (TIBC)-in the same serum specimen may facilitate the diagnosis of iron deficiency or overload [1-3].Serum ferritin level generally reflects body iron storage and can be used in the diagnosis of iron deficiency and overload. Transferrin saturation is the percentage of iron bound to transferrin. In patients with anemia, ferritin levels are most frequently used to determine whether iron deficiency is the cause [1]. However, as an acute phase protein, ferritin levels can be increased independently of iron status in inflammatory conditions, kidney disease, liver disease, and malignancy. In these clinical scenarios, the combination of ferritin level with other tests, such as transferrin saturation, may aid in the evaluation of iron deficiency [1,3].In patients with suspected hemochromatosis, transferrin saturation and serum ferritin may be included in the initial evaluation of iron overload [2]. The combination of a transferrin saturation under 45% and a normal serum ferritin level may help exclude iron overload. A transferrin saturation equal or over 45% alone or with an elevated ferritin level may suggest further testing. Serum ferritin level is also a predictor of advanced fibrosis [2].Note that reference intervals of serum iron, TIBC, transferrin saturation, and ferritin depend upon age and sex.The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1. Description:
IDH1 and IDH2 (Isocitrate dehydrogenase 1 and 2) encode enzymes that catalyzes oxidative decarboxylation of isocitrate into alpha-ketoglutarate (alpha-KG). Gain-of-function mutations in the IDH genes are associated with aberrant conversion of alpha-KG to 2-hydroxyglutarate (2-HG), which is an oncogenic metabolite, and are recurrent in myeloid malignancies and glioma. IDH1 (7-14%) exon 4 and IDH2 exon 4 (8-19%) mutations occur frequently in AML and are found to be associated with unfavorable prognosis and inferior overall survival in some studies. IDH inhibitors have been FDA-approved for IDH-mutated relapsed or refractory acute myeloid leukemia (AML).

This test detects the presence of specific IgE against toluene diisocyanate (TDI). A positive test only indicates that a person is sensitized and potentially allergic to this allergen. A diagnosis of allergy requires clinical signs and symptoms that appear to be associated with exposure to the allergen in addition to the detection of markers of IgE sensitization [1].More specific information about this allergen can be found on the following website: https://www.thermofisher.com/phadia/us/en/resources/allergen-encyclopedia.htmlReference1. Description:
This test quantifies an individualโs IgE response to Italian cypress. This in vitro quantitative assay is intended to be used in conjunction with other clinical information to aid in the diagnosis of allergic diseases [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. A definitive clinical diagnosis of allergy should not be based on the results of any single diagnostic method, but should be made by a trained healthcare professional after all clinical and laboratory findings have been evaluated. Description:
The antifungal drug itraconazole is active in the treatment of aspergillosis, blastomycosis, histoplasmosis, sporotrichosis, and onychomycosis. It is a protein-bound drug and thus has negligible CSF penetration. It may also be utilized in the treatment of systemic candidiasis and cryptococcosis, especially when other drugs are contraindicated or ineffective. The chronic nature of fungal infections demands constant monitoring of itraconazole levels within a patient to ensure that adequate therapeutic levels of the drug are administered, absorbed and subsequently excreted

Description:
This test is an allergen-specific IgE antibody test that quantifies an individualโs IgE response to jack Mackerel/scad. It is an in vitro quantitative assay, which is intended to be used in conjunction with other clinical information to aid in the diagnosis of food allergy [1]. Food-specific IgE tests are extremely sensitive. However, a positive test result only indicates that a patient is IgE sensitized to the food of concern. Many IgE-sensitized patients do not develop any symptoms when this food is ingested. A diagnosis of food allergy should only be made by a trained medical provider, after conducting a thorough clinical evaluation [2,3]. While food-specific IgE test results may contribute to that evaluation, they cannot replace it. In this regard, detection of food-specific IgE in serum provides evidence of IgE sensitization, but a history of clinical reactivity to the food of concern, is required to make a diagnosis of IgE-mediated food allergy. Moreover, several forms of food hypersensitivity are not associated with the presence of food-specific IgE in serum. Description:
JC polyoma virus is the cause of progressive multifocal leukoencephalopathy (PML), a demyelinating neurologic disease of immunosuppressed patients. This test detects JC Polyoma Virus DNA in serum or plasma. Detection of virus in specimens from AIDS and other severely immune compromised patients may indicate disease. Serum antibodies are not helpful in diagnosing PML as antibodies are common in the general population. JCV can also be detected in the urine of 3% of pregnant women, but is not associated with proven disease.
