Description:
Measurement of albumin levels in urine below the detection level of urine dipsticks. This test is useful in the management of patients with relatively early diabetes mellitus to assist in avoiding or delaying the onset of diabetic renal disease.
Microalbumin is albumin excreted in the urine and is a sensitive marker of nephropathy. It is used to screen for early renal disease in diabetic patients.
Creatinine is the endproduct of creatine metabolism. Creatine is present primarily in muscle and the amount of creatinine produced is related to total skeletal muscle mass. Daily creatinine production is fairly constant except when there is massive injury to muscle. The kidneys excrete creatinine very efficiently and blood levels and daily urinary excretion of creatinine fluctuates very little in healthy normal people. Since blood and daily urine excretion of creatinine shows minimal fluctuation, creatinine excretion is useful in determining whether 24-hour urine specimens for other analytes (e.g., protein) have been completely and accurately collected.
A significant deviation from the normal range may require further evaluation by your physician.
Includes:
Microalbumin, Random Urine with Creatinine
Patient Instructions:
Exercise within 24 hours, infection, fever, congestive heart failure, marked hyperglycemia, and marked hypertension may elevate urinary albumin excretion over baseline values.
Estimated turnaround for results is 4 business days.
If confirmation testing is required, the estimated time may be extended.

Description:
Copper is an essential element that is a cofactor of many enzymes. Copper metabolism is disturbed in Wilson's disease, Menkes disease, primary biliary cirrhosis, and Indian childhood cirrhosis. Copper concentrations increase in acute phase reactions and during the third trimester of pregnancy. Copper concentrations are decreased with nephrosis, malabsorption, and malnutrition. Copper concentrations are also useful to monitor patients, especially preterm newborns, on nutritional supplementation. Results of copper are often interpreted together with ceruloplasmin.

The best known of these are the antioxidant glutathione peroxidase enzymes, which remove hydrogen and damaging lipid and phospholipid hydroperoxides generated in vivo by free radicals and other oxygen-derived species. If not removed, lipid hydroperoxides impair membrane structure and function and cause blood-clotting disturbances by decreasing the production of prostacyclin while increasing the production of thromboxane.

Measurement of human lymphocytes' proliferative responses to various stimuli is a fundamental technique used to assess their biological status and functions. Mitogens, such as plant lectins phytohemagglutinin (PHA), concanavalin A (Con A) and pokeweed mitogen (PWM), are able to nonspecifically stimulate lymphocyte proliferation and used to evaluate patient immune responsiveness. Lymphocyte proliferation response to antigens, such as Candida, tetanus toxoid and tuberculin purified protein derivative (PPD), are evaluated as a function of memory in cell-mediated immunity.

Mixed Connective Tissue Disease Panel (RNP Antibody and ANA with Reflex)
*Build Your Own Panel Eligible [?]

|
Description:
This panel can be helpful in the diagnosis of individuals with clinical suspicion of mixed connective tissue disease (MCTD). All specimens are tested for antinuclear antibodies (ANAs) with an immunofluorescence assay (IFA), performed on human epithelial type 2 (HEp-2) cells; and RNP antibodies with a multiplex immunoassay. For specimens with positive ANA IFA results, reflex testing for ANA titer and pattern is performed. The laboratory evaluation for individuals with clinical suspicion of autoimmune diseases often begins with an ANA screen. The classic ANA IFA on HEp-2 cells is considered by the American College of Rheumatology (ACR) as the current gold standard because of its overall high sensitivity for several autoimmune diseases [1]. Knowing the ANA titer and fluorescent staining pattern can be helpful in interpreting positive results [2]. The first indication of MCTD is often a high ANA titer (>1:1000), which occurs in 94% to 97% of patients with MCTD [3]. The presence of RNP antibodies is considered the serologic hall mark of MCTD and required by diagnostic criteria [3].Individuals with negative results on the ANA IFA usually also have negative results on specific autoantibodies. Therefore, subserology testing is not recommended for individuals without positive ANA IFA results and clinical suspicion of relevant autoimmune disease [4]. However, Jo-1 antibody may be detected in ANA IFA-negative patients with some types of myositis, and SSA antibody may be detected in some ANA IFA-negative patients with lupus or Sjogren syndrome [4].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1. Mixing Study

Description:
This test is used to help differentiate coagulation factor deficiency from inhibitor activity in patients with prolonged clotting times-prothrombin time (PT) and activated partial thromboplastin time (aPTT).Deficiency of a coagulation factor or factors can be congenital or acquired. Congenital factor deficiency is caused by pathogenic variants of the gene that encodes the coagulation factor; acquired factor deficiency is caused by inhibitors that interfere with factor activity. This mixing study can be used to determine the cause of factor deficiency. If the clotting time of the immediate mix remains prolonged ("not corrected"), the presence of an inhibitor is suspected (ie, lupus anticoagulant, factor specific inhibitors, anticoagulant, or other non-specific inhibitors). If the clotting time of the immediate mix is below the cut-off ("corrected"), a factor deficiency is suspected. Because some inhibitors (eg, inhibitors against factor VIII) can only be detected after incubation, an incubated mix may also be included with the aPTT mixing study [1].This test can be used to guide the investigation of coagulation disorders but has limited sensitivity and specificity [1], specifically if the baseline clotting test is minimally prolonged. Tests for coagulation factors, specific inhibitors, and lupus anticoagulants may be needed for the diagnosis.Note that the test results may be affected by the titer and strength of inhibitors, number of the deficient coagulation factors, heparin contamination, anticoagulants, direct thrombin inhibitors, or direct Xa inhibitors [1].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.Reference1. Description:
This immunohistochemistry (IHC) assay is based on an antibody directed against the DNA mismatch repair (MMR) protein MLH1. MLH1 IHC testing is usually done as part of a panel that assesses expression of 4 MMR proteins (the other 3 are MSH2, MSH6, and PMS2) in resected tumor tissue from patients with newly diagnosed colorectal or endometrial cancer. Loss of expression of 1 or more of these proteins may indicate defective DNA repair processes associated with Lynch syndrome [1,2].IHC for MMR proteins is generally performed on tissue from certain tumors (eg, colorectal carcinomas and endometrial carcinomas) to screen for Lynch syndrome. IHC testing of MMR proteins identifies approximately 95% of tumors with defective MMR [1]. DNA microsatellite instability (MSI) is another testing methodology that may help screen for patients with Lynch syndrome. MSI-high results and/or loss of expression of any MMR protein (except MLH1) can be followed by germline pathogenic variant testing of the relevant genes to confirm Lynch syndrome [1].Loss of MLH1 expression may reflect germline mutation in the MLH1 gene (in the setting of Lynch syndrome) or MLH1 promotor methylation and BRAF mutation (in the setting of sporadic tumorigenesis). Therefore, in patients with loss of MLH1 expression, testing tumor tissue for a BRAF mutation or for hypermethylation of the MLH1 promoter may be performed before proceeding to germline genetic testing for Lynch syndrome. In patients with high suspicion of Lynch syndrome, genetic testing should be considered despite normal screening results [2].The results of IHC of MMR proteins and MSI may also be useful in evaluating chemotherapy and immunotherapy choices for many solid tumors. The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1.
