Description:
The clinical manifestations, severity, and prognosis of nocardiosis are extremely variable. In immunocompetent hosts, localized subcutaneous infections are the most common infections. N. brasiliensis is the predominant causative agent of primary cutaneous infections. In immunocompromised patients, the most common clinical presentations are invasive pulmonary infections and disseminated disease. These infections are produced by several species including Nocardia farcinica and Nocardia nova.Clinical symptoms of Nocardiosis may resemble tuberculosis or actinomycosis. Nocardia disease, especially in immunocompromised patients may begin as a pulmonary infection and then disseminate to the central nervous system, kidneys, and other organs. On rare occasions, the eye has been infected.

Mountain Cedar (t6) IgE

Description:
This test quantifies an individual’s IgE response to mountain juniper. It is an in vitro quantitative assay, which is intended to be used in conjunction with other clinical information to aid in the diagnosis of allergic diseases [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. A definitive clinical diagnosis of allergy should not be based on the results of any single diagnostic method, but should be made by a trained healthcare professional after all clinical and laboratory findings have been evaluated. More specific information about this allergen can be found at http://www.phadia.com/en/Products/Allergy-testing-products/ImmunoCAP-Allergen-Information/Tree-Pollens/Allergens/Mountain-juniper-/ Reference 1. Description:
Helps physicians diagnose and predict the course of disease progression for hematologic diseases. In the updated WHO Classification of Hematologic Malignancies (2016), the major diagnostic criteria for several different myeloproliferative neoplasms require the presence of specific gene mutations with supporting clinical and morphologic features. The JAK2 V617F mutation is found in virtually all cases of Polycythemia Vera (PV), with rare cases harboring a JAK2 exon 12 mutation. Mutations in JAK2 V617F, MPL codon 515 and CALR exon 9 are also major diagnostic criteria for Essential Thrombocythemia (ET) and Primary Myelofibrosis (PMF). Activating mutations in CSF3R (e.g. T618I) are now included as diagnostic criteria for Chronic Neutrophilic Leukemia (CNL). Furthermore, SETBP1 mutations are found in a subset of BCR-ABL1 negative, atypical Chronic Myeloid Leukemia, (aCML) cases that lack mutations in JAK2, MPL, and CALR. The release of the v. 2.2017 NCCN Guidelines for MPNs further recommend testing for gene mutations in ASXL1, EZH2, IDH1, IDH2 and SRSF2 for risk stratification in primary myelofibrosis.

Description:
Reduced methylenetetrahydrofolate reductase (MTHFR) enzyme activity is a genetic risk factor for hyperhomocysteinemia, especially when present with low serum folate levels. Two common variants in the MTHFR gene result in reduced enzyme activity. The "thermolabile" variant C677T [NM 005957.3: c.665C>T (p.A222V)] and A1298C [c. 1286A>C (p.E429A)] occur frequently in the general population. Mild to moderate hyperhomocysteinemia has been identified as a risk factor for coronary artery disease and venous thromboembolism. Hyperhomocysteinemia is multifactorial, involving a combination of genetic, physiologic and environmental factors. Recent studies do not support the previously described association of increased risk for coronary artery disease and venous thromboembolism with mild hyperhomocysteinemia caused by reduced MTHFR activity. Therefore, the utility of MTHFR variant testing is uncertain and is not recommended.

Description:
Detects anti-MuSK (muscle specific receptor tyrosine kinase) and anti-LRP4 (low density lipoprotein receptor-related protein 4) in the sera of patients with Myasthenia Gravis (MG). MuSK is a transmembrane endplate polypeptide involved in a signaling pathway that maintains the normal functional integrity of the neuromuscular junction (NMJ). The LRP4 protein activates MuSK kinase activity and promotes the clustering of the acetylcholine receptor (AChR) and its stabilization at the NMJ. Approximately 8% to 10% of all MG cases and 20% to 50% of the generalized MG cases who lack anti-AChR have anti-MuSK. Anti-LRP4 antibodies are detected in the serum of approximately 9.2% (range 2% to 50%) of MG patients who are negative for both anti-AChR and anti-MuSK.

Anti-mutated citrullinated vimentin (MCV) antibody is one of several specific anti-citrullinated protein antibodies which may be used as a second-line marker of rheumatoid arthritis (RA), in addition to rheumatoid factor and anti-cyclic citrullinated peptide (CCP). MCV antibody is less specific than CCP but has a slightly higher sensitivity and the addition of MCV to CCP adds 5-10% sensitivity for the diagnosis of RA. MCV antibody positivity is also associated with more severe disease and greater radiologic progression.
