Description:
Since the mid-1980s and expanding size of the immunocompromised patient population, the incidence of septicemia caused by opportunistic pathogens such as yeast, fungi and mycobacteria has risen. Mycobacterium tuberculosis (MTB) complex and mycobacteria other than tuberculosis (MOTT), especially Mycobacterium avium complex (MAC), have become resurgent. The Centers for Disease Control and Prevention (CDC) have recommended that every effort must be made for laboratories to use the most rapid methods available for diagnostic mycobacteria testing. These recommendations include the use of a liquid medium for mycobacterial culture. The BD BACTEC fluorescent series instruments are designed for the rapid detection of microorganisms in clinical specimens.

Description:
Globally, about 2 billion people are infected with MTB. In 2010, 8.8 million people developed active disease and 1.4 million people lost their lives to the illness. There were 9951 new cases of tuberculosis reported in the United States in 2012 (a rate of 3.2 cases per 100,000). In 2011, 536 deaths were attributed to tuberculosis infections.Standard treatment regimens for tuberculosis involve prolonged administration of multiple drugs and is usually highly effective. However, MTB-complex strains that are resistant to one or more of first line drugs require individualized treatment. Rifampin resistance is often an indication of multidrug resistance to tuberculosis (MDR TB), which is defined as resistance to at least rifampin (RIF) and isoniazid (INH). In the Unites States overall RIF resistance is approximately 1.8% with approximately 90% of these strains resistant to at least RIF and INH.

All patients in healthcare facilities with suspected TB should be maintained in respiratory isolation according to recommended infection control guidelines. Patients with HIV infection and pulmonary TB are more likely to have sputum samples that are acid-fast bacilli (AFB) smear negative than patients with pulmonary TB alone. Overall rates of detection of MTB-complex with the Xpert (TM) MTB/RIF Assay may be lower in settings with a high percentage of HIV-infected patients relative to populations where the percentage of patients with HIV infection is lower and AFB smear-positive TB is higher.

Description:
This test is for the detection of Mycoplasma genitalium and macrolide resistance markers in M. genitalium positive samples. Macrolide resistance testing may be useful for resistance-guided therapy in patients with M. genitalium infection. A two-stage therapy approach which includes doxycycline as the initial empiric therapy and follow-up treatment based on results from resistance testing is recommended.M.genitalium is a sexually transmitted bacteria that can cause urethritis in men and cervicitis and pelvic inflammatory disease in women. Resistance to macrolides has been increasing and contributes to treatment failure, although rates of resistance vary by geographical region and patient population. Doxycycline is recommended as the initial empiric therapy, followed by azithromycin for patients with macrolide sensitive M. genitalium infections or moxifloxacin for macrolide resistant infections. This approach is aimed at reducing organism load and facilitates clearance, ultimately decreasing the risk of treatment failure in infected patients.A detected macrolide resistance result for this test indicates that the organism contains one of the characterized mutations (A2056G, A2058C, A2059G, A2059C, A2058T) associated with the resistance to macrolides. Although a not detected result may suggest a macrolide sensitive infection, macrolide resistance may still be present even when markers are not detected due to differences in analytical sensitivity between the detection of M.genitalium nucleic acid and markers of resistance. Analytical sensitivity may also vary based on the specific mutation associated with the macrolide resistance.References:1. Wood, GE, et al. Description:
The Aptima Mycoplasma genitalium assay is an in vitro nucleic acid amplification test (NAAT) for the qualitative detection of ribosomal RNA (rRNA) from Mycoplasma genitalium. It is intended for use as an aid in the diagnosis of M. genitalium urogenital infections in male and female patients suspected of M. genitalium infection. Infection with M. genitalium was shown to be strongly associated with non-gonococcal urethritis (NGU) in men and cervicitis in women. Infection with M. genitalium is associated with increased risk for cervicitis, pelvic inflammatory disease, preterm birth, spontaneous abortion, and infertility. M. genitalium infections largely go unrecognized, as infected individuals are either asymptomatic or have symptoms similar to those associated with other bacterial infections of the urogenital tract.

Description:
Mycoplasma pneumoniae DNA, PCR, is a highly sensitive method to detect the presence of Mycoplasma pneumoniae DNA in clinical specimens. The diagnosis of Mycoplasma pneumoniae infection should not rely solely upon the result of a PCR assay. A positive result should be determined in conjunction with clinical presentation and additional established clinical tests. Moreover, this assay cannot differentiate between viable and nonviable organisms. A negative PCR result indicates the absence of Mycoplasma pneumoniae DNA at detectable levels in the sample tested and does not exclude the diagnosis of disease.

Description:
Mycoplasma pneumoniae is a widely distributed bacterium and causes primary atypical pneumonia, often called "walking pneumonia". M. pneumonia can be cultured on artificial media but may take 21-30 days for growth to appear. Alternatively, M. pneumoniae IgA antibodies may be used for rapid and specific identification of this disease.A negative serological result does not exclude infection, as in early phase of infection, antibodies may not be present, or may be too low to be detectable. A borderline result indicates a solid (negative or positive) result was not obtained. A positive result indicates contact with the bacterium, Mycoplasma pneumoniae.

Description:
Mycoplasma and Ureaplasma spp. have been associated with genital tract infection. M. genitalium has been implicated in cervicitis, endometritis, and PID. M. hominis has also been associated with cervicitis and is present in increased numbers in vaginosis. Ureaplasma spp. have been linked to upper genitourinary tract infection and to premature labor and abortion. Because of their nature, these organisms are not routinely cultured and are better detected by molecular methods. Recently, by molecular means two groups of Ureaplasma spp. have been distinguished- U. parvum and U. urealyticum, with the former being more prevalent in the lower genital tract of a healthy woman. It is not clear which species more predominantly causes infection. M. hominis and the ureaplasmas appear to be opportunists when they infect the upper genitourinary tract. These organisms have also been targeted as having a possible role in infertility, although this remains a gray area.

Description:
Oncogenic mutations in MYD88, in particular the L265P mutation, have been identified as driver mutations in various B-cell lymphomas. The L265P mutation is found in approximately 90% of Waldenstom macroglobulinemia and IgM-expressing lymphoplasmacytic lymphoma (LPL). There is a low incidence of L265P MYD88 mutation in other systemic CD5-negative B-cell lymphoproliferative disorders including atypical chronic lymphocytic leukemia, nodal marginal zone lymphoma (MZL), splenic MZL and mucosa-associated lymphoid tissue (MALT)-type MZL. This next-generation sequencing (NGS) based assay detects clinically significant MYD88 mutations, including the L265P mutation and can be used to help diagnose Waldenstrom macroglobulinemia or IgM-expressing lymphoplasmacytic lymphoma and to help in stratifying or subclassifying patients with IgM monoclonal gammopathy.

Description:
The MPN Core Diagnostic panel is an evidence-based, disease targeted mutational analysis panel, performed by Next-generation sequencing (NGS). Testing for disease-defining driver mutations in JAK2 V617F, JAK2 exon 12, CALR, and MPL, as part of the early evaluation for BCR-ABL1-negative myeloproliferative neoplasms (MPNs) including polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF), assists with diagnosis; is supported by medical guidelines; should be performed using highly sensitive molecular methods; and also impacts prognosis and therapy selection. The panel tests nucleic acid harvested from leukocytes from blood or bone marrow specimens. This panel does NOT include testing for the BCR-ABL1 rearrangement, which can be detected using BCR-ABL1 Gene Rearrangement, Quantitative, PCR or FISH, CML/ALL, BCR/ABL Translocation 9;22. MPN patients who are found to be pan-negative (~10% of cases) on the MPN Core Diagnostic panel may be candidates for CSF3R Mutation Analysis or Quest?s LeukoVantage®, Myeloproliferative Neoplasms (MPN), which offers an expanded panel of gene targets supported by medical literature. Results of this assay should be correlated with morphology and other clinical and laboratory findings for definitive diagnosis and classification.

Description:
The MPN mini profile detects disease-defining, potentially targetable mutations in JAK2, CALR, and MPL for myeloproliferative neoplasms. These 3 genes are growth- promoting oncogenes that operate through a signaling pathway mediated by JAK2, and MPNs with these mutations can respond to clinically available JAK2 inhibitors. The inclusion of hot spot mutation testing in these 3 genes into the routine workup of MPNs is recommended by the World Health Organization's classifications of hematological malignancies and the National Comprehensive Cancer Network (NCCN) guidelines. This combined testing method provides diagnostic assistance, provides a clonal molecular marker of disease following diagnosis and post therapy follow-up, influences prognosis, and identifies those MPNs that may potentially benefit from JAK2 inhibitor therapy.

Description:
Myositis-specific autoantibodies (MSAs) are highly selective, usually exclusive and are associated with particular clinical phenotypes within the myositis spectrum. Myositis-specific autoantibodies to cytoplasmic enzymes that catalyze the binding of specific amino acids to their cognate tRNA define the anti-synthetase syndrome characterized by myositis and lung inflammation, and include autoantibodies to Jo-1 (antihistidyl-tRNA synthetase), PL-7 (threonyl), PL-12 (alanyl), EJ (glycyl), and OJ (isoleucyl). A further subset of myositis patients is characterized by the presence of autoantibodies directed against the signal recognition particle (SRP). Autoantibodies to Mi-2 (Mi-2/nucleosome remodeling and deacetylase (NuRD) complex) are detected in patients with hallmark dermatomyositis features. Autoantibody to a cytoplasmic 140-kDa protein, melanoma-differentiation associated gene 5 (MDA5), also known as anti-CADM140, identified patients with clinically amyopathic dermatomyositis (CADM) and rapidly progressive lung disease. Autoantibodies to transcriptional intermediary factor 1-gamma (TIF-1 y), a p155/140 nuclear protein involved in cellular differentiation, have been reported in adult and juvenile dermatomyositis; in adults, it is associated with malignancy. Autoantibodies to nuclear matrix protein NXP-2, another 140-kDa protein, is found in juvenile dermatomyositis (JDM). Anti-TIF-1 y and anti-NXP-2, two novel serological subsets in JDM, occur collectively in >40% of children and appear to identify those with more severe disease.

A two-fold or greater increase for at least two serogroups is expected when comparing post-vaccination to pre-vaccination results. N. Meningitidis IgG levels peak approximately one month post-vaccination, but decline markedly by two years.
