TSH with Reflex to Free T4

Description:
This test may be useful in assessing thyroid dysfunction when pituitary disease is not suspected. In patients with clinical suspicion of hyperthyroidism or hypothyroidism, testing thyroid stimulating hormone (TSH) is the initial step [1]. An abnormal TSH result will reflex to a free thyroxine (T4) test to aid in diagnosis and guide further testing if needed.TSH stimulates the thyroid gland to synthesize and secrete triiodothyronine (T3) and T4. TSH production is reduced in response to high T3/T4 levels and increased in response to low T3/T4 levels. When pituitary disease is not suspected, TSH serves as a sensitive marker for screening for thyroid dysfunction [1,2]. A normal TSH result excludes most cases of primary overt thyroid disease. When the TSH level is elevated, measurement of free T4 level may help diagnose subclinical or overt hypothyroidism. Thyroid peroxidase antibody testing may be needed to aid in the diagnosis of Hashimoto thyroiditis. When the TSH level is decreased, measurement of free T4 and free T3 may help identify hyperthyroidism or T3 thyrotoxicosis. In patients with thyrotoxicosis, TSH receptor antibodies testing helps confirm Graves disease [1-3].Note: Interference due to heterophile antibodies has been known to occur [1].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1. Turkey Feathers (e89) IgE

Description:
This test quantifies an individual’s IgE response to turkey feathers. It is an in vitro quantitative assay, which is intended to be used in conjunction with other clinical information to aid in the diagnosis of allergic diseases [1]. While allergen-specific serum IgE testing is considered comparable to skin testing in many instances, both the American Academy of Allergy, Asthma, and Immunology and the American College of Allergy, Asthma, and Immunology recognize that allergen-specific serum IgE testing may be preferred in some clinical situations. These include 1) the presence of widespread skin disease, 2) the recent use of antihistamines or other medications that can affect the results of allergy skin tests, 3) uncooperative patients, and 4) medical history suggesting that allergen skin testing would pose a significant risk for a serious allergic reaction [1]. A definitive clinical diagnosis of allergy should not be based on the results of any single diagnostic method, but should be made by a trained healthcare professional after all clinical and laboratory findings have been evaluated. More specific information about this allergen can be found at http://www.phadia.com/en/products/allergy-testing-products/immunocap-allergen-information/epidermals-and-animal-proteins/allergens/turkey-feathers/ Reference 1. Description:
This test quantifies an individual’s IgE response to turkey meat. It is an in vitro quantitative assay, which is intended to be used in conjunction with other clinical information to aid in the diagnosis of food allergy [1]. Food-specific IgE tests are extremely sensitive. However, a positive test result only indicates that a patient is IgE sensitized to the food of concern. Many IgE-sensitized patients do not develop any symptoms when this food is ingested. A diagnosis of food allergy should only be made by a trained medical provider, after conducting a thorough clinical evaluation [2,3]. While food-specific IgE test results may contribute to that evaluation, they cannot replace it. In this regard, detection of food-specific IgE in serum provides evidence of IgE sensitization, but a history of clinical reactivity to the food of concern, is required to make a diagnosis of IgE-mediated food allergy. Moreover, several forms of food hypersensitivity are not associated with the presence of food-specific IgE in serum. Description:
This test estimates urea clearance based on the measurements of urea nitrogen levels in a blood specimen and a 24-hour urine specimen [1]. Urea clearance may be helpful in screening for chronic kidney disease and predicting glomerular filtration rate (GFR) in patients with advanced kidney dysfunction [1].Urea is the main nitrogen-containing product of protein breakdown, and approximately 90% of the urea excretion is through the kidneys. In individuals with normal kidney function, urea clearance is roughly half of creatinine clearance owing to tubular back-diffusion of urea to blood and making it a poor alternative to assess kidney function [1]. Averaging creatinine clearance and urea clearance is also not recommended for this purpose owing to its inferior performance [2]. However, in patients with advanced kidney dysfunction, as the urea back-diffusion decreases urea clearance may better reflect GFR [1].Blood urea levels, commonly expressed by the concentration of blood urea nitrogen (BUN), may be increased in individuals on a high-protein diet or with increased protein breakdown, gastrointestinal hemorrhage, chronic heart failure, or treatment with cortisol [2]. BUN level may be decreased in individuals with low-protein diet and end-stage liver disease [2].Urinary urea levels, commonly expressed by the concentration of urinary urea nitrogen, may be increased in individuals with hyperthyroidism or excess protein intake or breakdown [3]. Urinary urea nitrogen level may be decreased in individuals with malnutrition, kidney damage or insufficiency, low-protein and high-carbohydrate diet, or liver disease. Pregnant persons and healthy children may also have low urinary nitrogen levels [3].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1. Description:
This quantitative urea nitrogen test, performed with a 24-hour urine specimen, may help estimate nitrogen balance and determine protein need in patients in critical conditions or receiving intravenous administration of nutrition [1].Urea is the main nitrogen-containing product of protein breakdown and makes up over 75% of total nonprotein nitrogen excreted. Approximately 90% of the urea excretion is through the kidneys, and the rest is through gastrointestinal tract and skin. Because urea secretion is closely related to protein catabolism, urinary urea, commonly expressed by the concentration of urinary urea nitrogen, may be used to assess nitrogen balance and guide protein intake [1,2]. Blood and urinary urea levels were previously used as kidney function markers but are only considered useful in certain clinical scenarios; they have been generally replaced by creatine levels [1].Urinary urea nitrogen level may be increased in individuals with hyperthyroidism or excess protein intake or breakdown [3]. It may be decreased in individuals with malnutrition, kidney damage or insufficiency, low-protein and high-carbohydrate diet, or liver disease. Pregnant persons and healthy children may also have low urinary nitrogen levels [3].The results of this test should be interpreted in the context of pertinent clinical and family history and physical examination findings.References1. Urea Nitrogen (BUN)
*Build Your Own Panel Eligible [?]

|
|
|
Description:
The blood urea nitrogen (BUN) test is a test of how well your kidneys are working. It measures the amount of nitrogen in your blood. The nitrogen is present in a chemical called urea. Urea is a waste product produced as your body digests protein. Urea is carried by the blood to the kidneys, which filter the urea out of the blood and into the urine.
Urea is the principle waste product of protein catabolism. BUN is most commonly measured in the diagnosis and treatment of certain renal and metabolic diseases. Increased BUN concentration may result from increased production of urea due to (1) diet or excessive destruction of cellular proteins as occurs in massive infection and fevers, (2) reduced renal perfusion resulting from dehydration or heart failure, (3) nearly all types of kidney disease, and (4) mechanical obstruction to urine excretion such as is caused by stones, tumors, infection, or stricture. Decreased urea levels are less frequent and occur primarily in advanced liver disease and in overhydration.
A significant deviation from the normal range may require further evaluation by your physician.

Description:
Uric acid measurements are useful in the diagnosis and treatment of gout, renal failure, and a variety of other disorders including leukemia, psoriasis, starvation, and other wasting conditions. Patients receiving cytotoxic drugs may be monitored with uric acid measurements. Only a minority of individuals with hyperuricemia develop gout. An increased uric acid level does not necessarily translate to a diagnosis of gout. The therapeutic goal for uric acid-lowering therapy is to promote crystal dissolution and prevent crystal formation. This is achieved by maintaining a uric acid level of less than 6 mg/dL.
A significant deviation from the normal range may require further evaluation by your physician.
Includes:
Uric Acid, Serum
Patient Instructions:
Discuss discontinuing drugs causing increased uric acid concentration include diurectics, pyrazinamide, ethambutol, and nicotinic acid with your primary care physician before testing.
