The Kappa/Lambda Light Chains, Free with Ratio, Serum test measures the amount of kappa and lambda free light chains (FLCs) in the blood and calculates the kappa to lambda ratio. It is used to assist in the diagnosis of plasma cell disorders, including multiple myeloma, primary amyloidosis, monoclonal gammopathy of unknown significance (MGUS), Waldenström’s macroglobulinemia, and related disorders. It is also used to monitor disease activity and treatment [1].
Kappa and lambda light chains link with heavy chains to form immunoglobulins. In healthy individuals, most serum light chains are bound to heavy chains. However, excess plasma-cell proliferation associated with plasma-cell disorders can cause an excess of circulating FLCs and increase or decrease the K/L ratio [1].
This test is intended for patients clinically suspected of having hematologic disease, based on certain symptoms and medical conditions. These include bone pain, bone fractures, normocytic anemia, kidney disease, and recurrent infections suggestive of an underlying plasma-cell disorder [1].
Note that blood samples that are grossly hemolyzed or lipemic or contain high levels of circulating immune complexes (CICs) should not be tested, because of the unpredictable degree of nonspecific scatter these sample types may generate.

Patient Instructions:
You should fast for 12-14 hours before collection of the specimen. Fasting is defined as no consumption of food or beverage other than water for at least 12-14 hours before testing. Some tests in this panel may exhibit interference when sample is collected from a person who is consuming a supplement with a high dose of biotin (also termed as vitamin B7 or B8, vitamin H or coenzyme R). Patient should be cautioned to stop biotin consumption at least 72 hours prior to the collection of a sample. In patients receiving therapy with high biotin doses (ie, > 5 mg/day), no sample should be taken until at least eight hours after the last biotin administration. Folate assays of samples from patients receiving therapy with certain pharmaceuticals (eg, methotrexate or leucovorin), are contraindicated because of the cross-reactivity of folate binding protein with these compounds. For accurate test results, no ejaculation during the 24 hours preceding the test. Discuss discontinuing drugs causing increased uric acid concentration include diurectics, pyrazinamide, ethambutol, and nicotinic acid with your primary care physician before testing. Cortisol, Total assay not recommended when patient is on prednisone/prednisolone therapy due to cross reactivity with the antibody used in this assay. If using a testosterone cream please be sure you have not rubbed any into the antecubital area of your arm for the last 24 hours as it can give elevated results.

This non-pathogen-specific immune function assay detects cell-mediated immunity by measuring intracellular adenosine triphosphate (iATP) levels in stimulated CD4 lymphocytes. This test may be useful in assessing the immunosuppressive status of solid organ transplant (SOT) recipients to help predict the risk of infection and optimize immunosuppressive regimens [1,2]. The risk of infection after transplantation depends upon the epidemiologic exposures and the net state of immunosuppression [3]. Excessive immunosuppression increases the risk of infection; inadequate immunosuppression increases the risk of graft rejection. Because CD4 lymphocytes mediate cell-mediated immunity (CMI), quantification of CD4 activation may be helpful in assessing the immunosuppression status after SOT. Early responses of CD4 lymphocytes can be quantified by measuring iATP production in stimulated CD4 cells. By assessing CMI, this assay may help estimate the aggregate impact of multiple factors, such as the use of immune suppressants and underlying clinical conditions, on a patient's immune status [1,2]. This assay cannot be used as a diagnostic test for infection or graft rejection and does not replace diagnostic assays (eg, culture, viral load, and biopsy) to inform clinical decisions [2]. Without standardized strategies to simultaneously assess a patient's risk for infection and graft rejection, clinical judgment remains crucial for individualizing patient management. The value of this assay for assessing risk of infection [4] and rejection [4,5] in SOT recipients needs further study. Results are most accurate if testing is performed on the day of blood draw. Performing the assay ≥1 day after the draw may lead to falsely low iATP levels [6]. This in vitro allergen-specific IgE panel is used to quantitatively measure an individual's IgE response to alpha-gal, an allergy component, and 3 meats (beef, pork, and lamb) that commonly trigger alpha-gal-associated meat allergy. This IgE panel may be used in conjunction with other clinical information to aid in the diagnosis of alpha-gal syndrome, a severe allergic reaction to mammalian meat induced by past tick bites [1]. Most cases of alpha-gal syndrome occur in the southeastern United States because of the heavy presence of the lone star ticks. Other types of ticks also have been associated with alpha-gal syndrome in Europe, Australia, Asia, and the northern and western United States. The alpha-gal (ie. galactose-alpha-1,3-galactose) IgE test helps evaluate the etiology of meat allergies in patients with delayed onset of symptoms (3 to 6 hours after eating meat) [1,2]. IgE antibodies to alpha-gal are the likely mediator of anaphylactic reactions in individuals who develop hypersensitivities to beef, pork and/or lamb as adults [2]. The cN-1A Ab assay is a useful aid for the diagnosis of inclusion body myositis (IBM). The analytical sensitivity of this assay is 35-70%, based on published reports [1, 2]. In our validation study with 120 healthy adult subjects, 1.7% had a positive cN-1A Ab result and 6.7% had an equivocal result. However, published reports and our own internal studies suggest that patients with Sjogren’s syndrome, systemic lupus erythematosus, dermatomyositis (DM), or polymyositis (PM), are significantly more likely to have a positive cN-1A Ab result [1, 2, 3]. In contrast, patients presenting clinically with IBM rarely produce autoantibodies associated with a diagnosis of DM or PM [3]. Therefore, cN-1A Ab results must be interpreted in context with other details of the patient’s clinical evaluation.

Patient Instructions:
Patient should fast for 12-14 hours before collection of the specimen. Specimens should be collected in the morning since iron values decrease by 30% during the course of the day and there can be significant interference from lipemia. For accurate test results, no ejaculation during the 24 hours preceding the test. If using a testosterone cream please be sure you have not rubbed any into the antecubital area of your arm for the last 24 hours as it can give elevated results.

Detection of antibodies to gliadin, one of the major protein components of gluten, is a sensitive assay useful in diagnosing celiac disease. However, gliadin antibodies may be found in individuals without celiac disease; thus gliadin antibody assays are less specific than assays measuring antibodies to endomysium and transglutaminase. Recent work has revealed that gliadin-reactive antibodies from celiac patients bind to a very limited number of specific epitopes on the gliadin molecule. Further, deamidation of gliadin results in enhanced binding of gliadin antibodies. Celiac disease is caused by an immune response to gluten in genetically sensitive individuals. The diagnosis is largely based on a biopsy of the small intestine, but serologic tests also help support a diagnosis and may assist identification of patients who may require biopsy.
Tissue transglutaminase antibodies (tTG, IgA) is a marker with 95% sensitivity and specificity. Total IgA is measured because 2-3% of celiac disease patients are IgA deficient. Because tTG, IgA, and anti-Gliadin IgA tend to decrease in patients on a gluten-free diet, these markers are also used to assess dietary compliance.
The endomysial antibody (EMA, IgA) assay has high specificity for celiac disease and is used to confirm positive anti-tTG results.

Description:
IgE-assays are the best-studied and most accepted serologic tests for evaluating food allergy status. However, some clinicians also use IgG tests when evaluating individuals for food allergies, sensitivities, or tolerance. Food-specific IgG is generally believed to be a normal physiological response to food exposures. Therefore, many professional organizations, including the American Academy of Allergy, Asthma & Immunology, the Canadian Society of Allergy and Clinical Immunology, and the European Academy of Allergy and Clinical Immunology, recommend against using IgG testing to diagnose food allergies or food intolerances/sensitivities [1-3].
